Patient Registration
At the Schroeder Ambulatory Centre, we want your visit to be smooth, safe and supported from the moment you arrive. See below for what floor to find registration on and important information on what to bring and expect:
Diagnostic Imaging: 1st Floor (L1) Registration A
Surgery and Procedures: 3rd (L3) Floor Registration B
Clinics: 5th Floor (L5) Registration C
What to Bring for Registration
When you register, please be prepared to provide the following:
- Your health card or another piece of government-issued photo ID
- The name, address, and phone number of your emergency contact
- You may also be asked to provide the name of your family doctor, along with their address and phone number
Your Safety Is Our Priority
To ensure the right care is provided to the right person, members of your healthcare team will ask for your name and date of birth at various points during your visit.

Accommodation requests are reviewed and managed on an individual basis. Please contact your clinical team in advance of your appointment or placement to discuss your accommodation needs and initiate your request.

Masking Policy
Masking is strongly recommended at Schroeder Ambulatory Centre. To be effective, masks should fully cover your nose, mouth, and chin.
If you have new cough or cold symptoms
please wear a mask to help protect others. You may also be asked to wear a mask by your care team during your visit. Masks are available at dispensers located near hospital entrances and elevators.
Public Notice of Behaviour:
Schroeder Ambulatory Centre is a safe space for everyone. We do not tolerate abuse including:
- Aggressive, threatening, or abusive language is not acceptable.
- Discrimination, bullying, or derogatory remarks will not be tolerated.
- Physical, sexual, or any form of harassment is strictly prohibited.
Anyone engaging in abusive behavior may be subject to appropriate actions, which could include involvement of security or further escalation. We ask everyone to help maintain a safe and inclusive environment by treating others with kindness, respect, and dignity.
