Patient Registration at Schroeder Ambulatory Centre

At the Schroeder Ambulatory Centre, we want your visit to be smooth, safe and supported from the moment you arrive. See below for what floor to find registration on and important information on what to bring and expect:

Diagnostic Imaging: 1st Floor (L1) Registration A
Surgery and Procedures: 3rd (L3) Floor Registration B
Clinics: 5th Floor (L5) Registration C 

What to Bring for Registration

When you register, please be prepared to provide the following:

  1. Your health card or another piece of government-issued photo ID
  2. The name, address, and phone number of your emergency contact
  3. You may also be asked to provide the name of your family doctor, along with their address and phone number

Your Safety Is Our Priority

To ensure the right care is provided to the right person, members of your healthcare team will ask for your name and date of birth at various points during your visit.

Accommodation Requests

Accommodation requests are reviewed and managed on an individual basis. Please contact your clinical team in advance of your appointment or placement to discuss your accommodation needs and initiate your request.

Masking is strongly recommended at Schroeder Ambulatory Centre. To be effective, masks should fully cover your nose, mouth, and chin.

If you have new cough or cold symptoms

please wear a mask to help protect others. You may also be asked to wear a mask by your care team during your visit. Masks are available at dispensers located near hospital entrances and elevators.
