Specialized geriatrics clinic services to support healthy and independent aging

The Geriatrics clinic at Schroeder Ambulatory Centre provides a comprehensive range of non-surgical services to help patients and caregivers navigate common issues that come up as we age. Led by leading Gerontologist Dr. Fabio Varlese, the clinic provides a variety of outpatient care programs for older adults who may be dealing with cognitive or physical challenges, as well as compassionate supports for caregivers. Our goal is to help older adults maintain independence, improve their quality of life and support ageing in place through personalized, coordinated care.

Appointments are available in person, virtually or by phone.

Schroeder Ambulatory Centre is currently working to establish a formal collaborative relationship with Baycrest, which would facilitate seamless referrals for patients to access programs and services available at both facilities. Our expert team of nurses, physicians and patient support staff work together with patients and families to develop care plans tailored to the individual, empowering them to age with dignity and in a way that promotes optimal cognitive function, physical health and independence.  

Our clinic is located in a brand new fully accessible 7-storey facility equipped with amenities and the most advanced technology for the diagnosis and treatment of aging issues. The Centre also has a full range of other clinics, including pain managementneurology, urology and primary care, all located on the same floor. When patients need more advanced care, including surgeries and diagnostic imaging, those are also available within the same building, ensuring a comfortable and easy patient experience.

Geriatrics

Schroeder Ambulatory Centre’s geriatrics clinic helps patients and caregivers deal with:

  • Cognitive decline (e.g. dementia, delirium, memory loss, confusion)
  • Functional decline (mobility issues, difficulty with daily tasks)
  • Increased falls
  • Dizziness / vertigo
  • Fainting, passing out and sudden loss of consciousness
  • Complex behavioural changes related to aging
  • Chronic illnesses
  • Safe and healthy use of multiple medications (polypharmacy)
    Frailty
  • Assessment and planning for home care or longterm care
    Care coordination
  • Repeat visits to the emergency room due to injury
  • Caregiver fatigue and burnout

Important information for geriatrics clinic patients

Our clinic is located on the 5th floor (L5) of Schroeder Ambulatory Centre. When you arrive, our clinic patient services team will help you check in to your appointment. Elevators are available near the main entrance, and where you enter from the parking garage.

If you require any assistance getting around, a member of our patient support team will be happy to assist! Just let us know what, if any, additional assistance you’ll need when you book your appointment.

Getting ready for your geriatrics appointment

  • Visit our Getting Here page to get directions, find parking and access other important information to ensure your visit goes smoothly.
  • Bring your Health Card and up to date list of medications, as well as any over-the-counter supplements you are currently taking
  • Bring any mobility aids (e.g. canes, walkers, wheelchairs)
  • Bring any devices you use to help you see or hear (e.g. glasses, hearing aids)
  • Bring any information you have about your previous medical history
  • Write down a summary of your symptoms and bring them to your appointment
  • If necessary or appropriate, bring friend or family member to help you before, during and after your appointment.
